Teamster Center Services.
In addition to our health insurance through MagnaCare, we have services available for substance abuse and for mental healthcare. The following is from your Summary Plan Description book (page 92).
In order to obtain benefits relating to mental healthcare and alchohol and substance abuse treatment, you should contact the Teamster Center Services (TCS) at (718) 920-5115. Inpatient admissions for alchohol and drug abuse or pyschiatric treatment must be made through the Teamster Center's referral in order for you to recieve maximum benefits. However, claims should be filed with the Fund Office.

These benefits are available to all of us and confidentiallity is the same as any other medical treatment/facility.If you have a substance abuse problem.
*NOTE* If you enter into a substance abuse program you are by contract required to see a substance abuse professional (SAP) and follow all guidelines set forth in the contract. These are too lengthy to discuss here but they are located in Article 35 (p.79) of your contract book. If you have questions you can speak to me privately, I can assure you that only the people who need to know will be involved.
Disability Claims
If you are hurt off the job and are not seeking workers compensation you must submit a completed claim form to the Fund Office. You should also request a withdrawal card for the time that you are out. This will not affect your medical benefits but you will not be responsible for union dues while you are out.
